Basic Requirements for Surrogates

  • Live in a surrogacy friendly state (check out this map)

  • Have successfully carried at least one child

  • Are raising/have raised at least one child

  • Your family is complete*

  • Be between 21-42 years old**

  • BMI under 33***

  • Healthy, complication-free pregnancy(ies) (must provide OB/GYN records)

  • No criminal history (must pass background check)

  • Be able to pass psychological screening

  • Be financially stable

  • No smoking, drug use, or alcohol abuse (able to pass drug screening)

  • Willing to have IUD or other birth control implant removed****

  • Be generally healthy and a good candidate for pregnancy! (You may be asked for a letter from you OB confirming this)

*This one isn’t 100% necessary, but it is strongly recommended. It *is* necessary that you are at least okay with your family as is. Any pregnancy can be a risk to your fertility, and IVF poses an even greater risk. You should not plan to be a surrogate “between this baby and the next one”, because there is a chance you will not be able to get pregnant or carry a baby again. If you know you want to have more children of your own, wait until your family is complete before pursuing surrogacy.
**The upper end of the age range varies by agency and by RE. Some have earlier cut-offs, and some will approve older women under some circumstances. 21 seems to be the standard minimum age.
***Different agencies and REs will have different BMI limits. Many will approve a surrogate with up to 35 BMI on a case-by-case basis.
****If you have an IUD or other form of implanted birth control, it will need to be removed and there is sometimes a waiting period (usually 3 month, particularly with an IUD) before you can cycle or sometimes even have your medical evaluation

As for IPs...

No one talks much about requirements to be an intended parent. This is probably because there are less instances of unfit potential intended parents than surrogates. However, there are some requirements for IPs, and I would like to mention them here.

  • Be able to pass psychological screening. This means:

    • Both partners (if you are partnered) must be on board with using a surrogate

    • Be emotionally stable and prepared for surrogacy (e.g. for giving up a lot of control and trusting your surrogate)

  • Have the finances to cover all necessary costs, including for potential complications

  • Be able to pass a background check
