The survey asked for the following information:

Agency Name

Agency Location

Respondent Type - Gestational Surrogate (GS), Traditional Surrogate (TS), Intended Parent (IP)

-Ratings (all out of 5)-

Overall Experience - 1=Hate them, 5=Love them

Communication - 1=Awful communication, 5=Great, fast communication

Matching Time - 1=Much longer than expected/average, 5=Super quick

Facilitation (of issues/negotiations between parties) - 1=They sucked, 5=They were awesome

Coordinator - 1=Terrible experience, 5=Best coordinator ever

Advocacy (on respondent's behalf w/match, clinic, atty, etc.) - 1=Felt like they didn't care about respondent, 5=Advocated fairly

*For IPs*: Agency Cost (compared to other agencies) - 1=More expensive, 5=Less expensive

*For IPs*: Value - 1=Wasn't worth the money paid, 5=More than worth the money paid

*For Surrogates*: Fee Package - 1=Much less than average, 5=More than average

Comments (anything else a respondent would like to say about the agency)
